(1) Their signature on a form prescribed by the Department. Except as provided in subsection (d), holders of a camera card shall go to a photo center to have their photograph taken and provide the following: (2) If a camera card is mutilated, lost, stolen, destroyed or becomes illegible, the person to whom it was issued shall obtain a replacement camera card upon payment of the required fee.
